Here's some things to check:

1) Improper nestling food.
2) Nestling food was spoiled
3) Mite and/or lice infestation -- spray the birds, nest, cage, and surrounding area with a avian pyrethrin insecticide.
4) Mice in the room disturbed the birds.
5) People bothering the birds
6) Improper lighting schedule
7) Lights or noises during the night disturbing the parents
